What to Bring to Your Appointment
We ask that you arrive for your first appointment 15-20 minutes early in order to complete our patient forms. Also, please bring
the following with you:

• Your insurance card
• Physician referral forms if required by insurance
• A list of current prescription or over-the-counter medications you are taking, including dose and frequency
• Pertinent information about your medical and surgical history
• Any pertinent test results/medical records/films you may have
